If you email Caremark, the automated e-reply you will receive is that Caremark is "working on" answering your question. (And then Caremark will "work on" this for a long, long time, and eventually you'll just give up. If you have Blue Cross insurance and ask how much your co-pay will be or what percent of the total cost for the vaccine your co-pay will be, the Blue Cross answer will be "it's complicated." Both Blue Cross and Caremark will avoid answering your direct question by very emphatically assuring you that the vaccine is "available" at "approved" pharmacies (even though your email or call is about cost and how much, not availability).
They didn't for my 50 year old wife. The pharmacy indicated that you may have to be over 60. The 2 pharmacies we checked were 215-250 dollars.
