If you're asking for the "in-universe" answer... the "Q" are eternal, so they have always existed since the universe began. But they didn't find it necessary to test humanity until Picard made his first voyage as captain of the Enterprise. The Q's motives are never entirely clear, but obviously they perceived humanity to be a growing threat to galactic peace and stability and therefore they decided to intervene.
As far as the "real-world" answer... Gene Roddenberry was an atheist, but nonetheless wanted humanity to be "judged" by some sort of omnipotent power and to be found worthy. The character of "Q" provided this, and also served as an example of how beings with absolute power could still be fallible.
