Break it down:
"America on Drugs." is the title of the article.
Newsweek is the title of the Magazine.
28 July 1989 is the date the magazine was published.
45-50 are the page numbers for the beginning and end of the articles.

You need the author's first and last name if the website lists it, the title of the web page, name of website, publisher is given, the web address (URL), the year published or revised if information is given, and the date accessed.

Yes it is. If you are wanting to add your own message in the card you can write on the left side of the card. Other than that it is proper to write on the right side of the card.

You can write an application to the director for an admit card by aquiring the necessary documentation and then submitting it to the person that it needs to be sent to.
