Same as with any railroad. There are maintence yards where the trains are above ground. New trains and old trains can be added and removed by crane from trucks or in some cases an interconnect track allows new cars to move directly onto the subway system tracks.

Take the MBTA commuter rail to South Station. Switchover to the MBTA red line and go inbound to Park Street. Change over to the Green Line and take any train EXCEPT the "E" train to Kenmore square. Fenway is a 1/2 mile walk from Kenmore.
New Jersey has its own train into Manhattan, called the PATH train, which connects to the Eighth Avenue subway line (the A-C-E, or the blue line). So, no, there are no plans to extend the MTA (Metro Transit Authority) subways into New Jersey.
