There's no way of telling. You can ride your heart out or you can coast along doing nothing much, it's all down to how hard you're riding. If you want to keep track, get a heart rate monitor, one of those that looks like a wrist watch and with a pickup strap that goes around your chest. It'll be far more reliable than any generic value presented to you by any machine that can't see how hard you're working.
Muddy question. In time you can obviously ride until you drop. In distance it all depends on terrain, weather, and rider fitness. 200 miles in a day is doable for a reasonably experienced rider.
Depends on why you're riding. If you're only riding for fun, ride until you're bored, hungry, cold or whatever that makes you think of quitting.
If you're riding because you need to get somewhere, better ride until you're there.
If you're riding to lose weight, basically ride for as long as you can. Aim for one hour at least

A long bike ride that is partially uphill be would be considered vigorous physical activity.
It cannot be done. To ride a bike around the earth, the continents would need to be connected, or the bike would need a really long bridge across the ocean.
