This is not an easy question. Drilling wells in deepwater requires doing everything just right, and then making the right decisions if things go wrong. For the Deepwater Horizon, a foamed cement was used to cement in the last sting of production tubing. There was inadequate testing of this cement. There was a test done just before the well blew out, that was showed the well was not completely sealed. The decision to continue to displace the drilling mud with sea water without completely knowing that the well was sealed up, allowed the oil to enter the well. It has also been shown that many last minute changes were made in the procedures to be carried out in the last few days before the accident. These changes results in many activities going on at the same time on the last day, and hindered the ability of supervisors to monitor the changes in the well. So, it wasn't just a single mistake, but a rather complicated series of events, or a combination of errors in design, equipment and human judgment that lead to the blow out. The National Commission on the Deepwater Horizon accident came to a number of conclusions on the changes to reduce the risks of oil spills. There report can be found on the internet, by searching under national commission on the deepwater horiaon accident. They felt the oil industry needed more supervision and regulation (oversight) to reduce the risk of deepwater drilling oil spills. A final report should be out by July 27. This is not the National Commission, but the Joint Investigation team, involving the US Coast Guard and the Minerals Management Service (MMS). The MMS has now changed its name. They will provide more information on the failure of the blow out preventer, which is supposed to allows shut in the well even if a number of mistakes are made in the cementing ang testing.
