If you have a hole in your ceiling and want to fix it you use a piece of wood to hold a piece of Sheetrock which you will finish with plaster or joint compound.
You would cut a piece of wood more narrow than the hole and longer than the hole.
Put the wood into the hole so it cannot fall out. Cut a piece of Sheetrock to about match the hole and screw the new piece of Sheetrock to the wood.
Use joint compound to give the repair a finished look. If you simply put joint compound into the hole it will crack and take many coats.

You can tile over existing tile although it isn't reccomended. You would be safer by removing the tile and then installing the new tile. But, if you wanted to tile over existing tile you would have to make sure all the tile is even and not lose. If the tile is lose or uneven it will caused the new tile and grout to crack or pop out.
It is best to remove the vinyl tile. If it is not on cement and is on wood you can install cement backer board. Be sure to put thin set under the board. You should NEVER install tile over laminate. Pull up the laminate (which is very easy) and install the appropriate subfloor, then install tile on that

A drywall ceiling, provides the best type of ceiling for work. It provides a great look, noise level reduction versus a tile ceiling and is inexpensive to install.
I would tile up to the pan where it sits. If you remove it, you will have to tile completely under it so it stays level and raise the drain. If it is in the basement or or over a finished ceiling this can be a problem. If there is a vanity, I would remove it and tile under because is is much less hassle and gives a better result.

If I am correctly understanding the question. You have a suspended ceiling and wish to add a register. Ok, two ways, the first is easiest. Get a 2`x 2` register and lay it in the grid in the place of a tile. Second, once you have selected the ceiling location for the register, lay the register in the center of the tile where it is going and trace out & cut the hole in the tile to accept the register and reinstall the tile into the ceiling grid and connect the duct. Make sure everything is properly supported and there is no stress on the tile.
