Medications that are designed to go into the system need to go through the digestive system. the mouth is the beginning of the digestive system and there fore it makes sense to put the pill in your mouth at the beginning, rather then shove it up the anus where there is no more digestion.

It depends on the type of medication and the recommended route of administration. Some meds would never break down if put into the anus and rectum, no matter how long you tried to hold them in. Conversely, some meds are made specifically to dissolve in the rectum and would be disgusting to try to put into the mouth. Other meds are topical (they go on the skin), so neither anal-rectal nor oral would be okay for those meds.
