ancient Rome had three types of leaders. In the time of the Roman Kingdom, it had a King; in the time of the Roman Republic, it had two Consuls for the majority of the time, and a Dictator in times of emergency; and in the time of the Roman Empire, it had figures now generally referred to as 'emperors', who held various titles (i.e. Augustus and Princeps).

Presuming you mean what was his title, Ancient Rome was first ruled by Kings. The last Roman King was Tarquinius the Proud, who was deposed because he was a tyrant. After that Rome was a Republic, ruled by two elected officials, the Consuls, who were advised by the Senate. In urgent times, times of national threat, a Dictator with emergency powers could be elected, but only for a short time. Eventually, the Republic was destroyed and replaced by an Imperial government, with an Emperor. The first Emperor was Octavius Caesar, the adopted son of Julius Caesar, who although never bore the title Emperor, was the main figure in the removal of the Republic, and was dictator for life - Emperor in all but name - until his assassination.

It depends on the historical period. Rome was a monarchy for 244 years (753 BC-509 BC) and was ruled by the king. It was a republic for 482 years (509 BC- 27 BC) which was headed by two annually elected consuls. It was ruled by emperors for 503 years (27 BC-476 AD).
