German U-Boats were patrolling waters that Allied powers used. The Lusitania, a luxury liner, was attacked and sunk in these waters. The ship was completely under in 12 minutes, killing 1,119 or the 1,924 passengers aboard, including 114 Americans. This coupled with other acts of brutality, such as sinking of neutral American merchant vessels in these waters, performed by the Germans caused President Woodrow Wilson to declare war on April 6, 1917.
The Zimmerman note was discovered which stated from Germany to Mexico that if they (Mexico) would invade the US, Germany would be their ally. Germany figured this would keep the US from sending supplies to Europe to support England and France.

Lastly, When Germany reinstated their policy of unlimited submarine warfare against all nations; Wilson broke off all diplomatic relations with Germany. Two months later, President Wilson asked Congress to declare war on Germany.

The VP and a majority of Cabinet Officers again declare the President's inability . 5. Both houses by 2/3 majority declare that the president is incapable of performing hs duties 6. The VP continues as acting president. There are time limits given for each step and default actions if the time limits are not met. My understanding is that steps 3, 4 and 5 can be repeated.
