You may ask about synthetic morphine-like medications if needed in the hospital. Natural morphine compounds have more of a probability, in many cases, of causing allergic reactions, whereas synthetic compounds are a bit less likely to.
Warn your Dr. about the allergy you had and you can probably have a skin test done with a pssible alternative. There are some pain medications that do not irritate the mast cells( that release histamine, causing allergy) that can be used instead.
Prescription drugs that can cause the pupils to contract are drugs from the opioid family. Morphine and codeine-dionine drugs are in this group. Others are hydroxy-morphine, dinitrophenylmorphine, and benzylmorphine. These drugs are used as pain killers.
The two chemicals are almost the same. Another name for codeine is "methylmorphine," or basically a morphine molecule with a Methyl group on the #3 carbon. In the body, enzymes in the liver typically remove this methyl group to convert codeine into plain old morphine. Typically about 10% of codeine is converted to morphine. A typically dose of codeine for an adult might be 30mg. With a 10% conversion rate, that's about 3mg of morphine.
The first thing that you need to know is if it is a true allergy or simply a strong manifestation of the common opioid side effects. If you do have a true allergy to codeine, you could be allergic to some or all of the semi-synthetic preparations or you could be fine with them. The semi-synthetic codeines include hydrocodone, oxycodone, and any other -codone named medications. You should also be aware that drug allergies can begin at any time. While you may have taken hydrocodone without incident for years, you may find yourself having a life-threatening reaction to it at any time. Once you have a reaction to codeine or a synthetic preparation, you are at an increased risk of reacting to the other preparations. It would be wise to keep epinepherine and diphenhydramine available if you are going to test the waters with any others. If you find yourself allergic to codeine or the semi-synthetic preparations, you may want to talk to your doctor about prescribing a medication from the morphine group as allergies to codeine and morphine are generally unrelated.
It's technically possible for someone to exhibit an allergic response (hives, difficulty breathing) to codeine, but have no problem taking oxycodone. That said, the odds are probably against it, and as such should only be attempted at the discretion of a trained medical professional (with OTC antihistamines on-hand, just in case). Molecularly speaking, codeine and oxycodone are very similar. Codeine lacks the hydroxy group at the 14- position, and has a 7-8 double bond. Also, the 6- position of codeine contains an alcohol group instead of a carbonyl.
