It sounds like balanoposthitis, which is an inflammation of the 'foreskin' and penis of a dog (humans can get it too but we won't go into that.) At any rate, the dog needs to see a vet who will likely flush the foreskin out with water or a hydrogen peroxide mixture and may prescribe a mild inflammatory to keep down the pain and itching.
