I believe that is 15 PSI on a low pressure steam boiler
200 temp on resident water heater 180 is ok BUT in extreme cases over 225 is used and according to the ASME /NBBI 250 DEG @160 PSI is considered a low pressure boiler

More than likely it is restiction in your water heater.

Depends on the situation! It may be needed at only a low pressure. Otherwise it may be the result of reduced heat input or of the input of cold feed-water.
