Start by asking him. Is it no desire or is it not until he is married? Ask him what he wants. Sounds like you should see a counselor, whether you can talk him into it or not. Sit him down and have a long honest talk. Tell him why sexual relations are important to you. Tell him how you feel about him seeing a doctor (are you concerned for his health?). Tell him what you need in order to stay together, and ask what he needs of you. Are both of your needs reasonable? Don't back down if you decide to break up. You should not feel trapped in a relationship, that's really unhealthy.
