Check all pulleys for proper alignment. Check for a failed bearing in a pulley. Check for a locked up pulley.
Check to see if the cowl drain tube over the belt is disconnected. If it is the rain water running out pours directly onto the serp belt and can cause the belt to slip off.
You can do that by replacing the tensioner

There should be a diagram under the hood for that year.
Go to www.autozone.com and look under specifications, belt routing. On a 1989 Dodge Grand Caravan, the diagram is on the side of the engine where the S-belt is located.

Not sure ,but it costs $115 to change mine on my 1998 Dodge Durango.
It is the item in the serpentine belt that is the lowest and furthest forward.
One of the pulleys is out of alignment,usually it is the tension pully. Replace it and problem will go away.
According to Edmunds.com, 1984 was the year that Dodge first rolled out the Caravan. That year Chrysler had both the Voyager and the Caravan coming off the assembly line.
The belt tensioner is self-adjusting.
