There is not one particular drug that is effective. A number of different drugs, in several different classes, may be used depending on patient need and reaction. Bipolar patients most often receive a cocktail of drugs from the following types.

Mood Stabilizers

The various salts of Lithium are effective in stabilizing mood (Lithium Carbonate, Lithium Citrate, etc).

Various anti-convulsants drugs typically used in the treatment of Eplilepsy (a related disorder) also are effective mood stabilizers. (Carbamazepine, Valproic Acid, Oxcarbazepine, etc.)

Anti-Depressants

For patients who have significant problems with Depressive states that mood stabilizers alone cannot control.

Non-SSRI type anti-depressants are favored as SSRI types, though occasionally used in crisis states, have a significant risk of triggering manias. Buproprion is often the non-SSRI drug of choice.

Neuroleptics

Patients with severe manias, particularly those displaying psychotic features are often given Neuroleptic (anti-psychotic) clas drugs.

Typical neuroleptics such as Haloperidol are often administered to actutely manic patients.

Maintenance treament generally entails use of atypical neuroleptics (Risperidone, Olanzapine, Quetiapine, etc).
