Think of people you know or have heard about. The BIGGEST red flag in my experience is lack of empathy. The reason for this is because the part of their brain that should light up when having a feeling, doesn't light up. Research has proven that the frontal cortex where we think lights up. Narcissists, sociopaths, and psychopaths literally aren't wired to feel, thus your answer to why they continue their destructive behavior patterns and never change. You cannot change what you cannot acknowledge. There is an online video you can Google of a neuroscientist showing a brain scan of a narcissist and it is very informative.
