No single easy answer, needs to be diagnosed properly. Could be shift solenoids in the valve body, computer issue, who knows. Spend the money to get diagnosed before chasing ghosts and throwing parts at it that are not needed.

This is was a problem on Dodge 2500 trucks prior to 2003 (or 2004?). You have to steer like crazy to keep it on the road. I've notice the steering on these rigs to be a lot looser than on my F350 or my Chev 3500. There is an aftermarket kit you can get to fix it.
unfortunately, most likely a blown head gasket. possibly a leak in your oil cooler line if your lucky.sorry-happened to me-cost $2500 to fix-good luck
