Hi,
I'll answer this based on having taken out my cable four times, I think it cost about $100 at a Chevy dealer,
and the first was to replace exactly what you have. Yes you really need to replace it so the speedo reads correctly all the time, they don't sell just the end. You really should have a pair of ramps to get under the car a bit easier or have a place where you can put the car to have easy access. To replace the cable is a bit of a job but if taken slowly and methodically it comes easy. The trickiest part is to remove the instrument panel, first the plastic that holds it in then the panel and do not lose the screws. You need to have brought a lot of the cable into the engine compartment, just watch how the cable is routed, if you have a bit of spray silicone or Pam, it will help if you spray it on the cable where it goes through the grommet in the firewall, it make the job easier because you need to pull some in with the instrument panel. When you put the new cable in you might want to take the left front speaker out as you can reach the cable through there too. I wish I could help you but I'll have to help you this way. Good luck

There are several things you can do to repair your speedometer. You can replace the speedometer cable. You can replace the speed sensor in the transmission.
possible cable Remove from transmission and check if in one piece - if not remove speedometer head and replace cable to transmission and head
You need to replace the speed sensor. It sends a signal to the speedometer and also sends a signal to the transmission on when it should shift.
unscrew it from the transmission and unscrew it from the inst.panel
Town and country vans have never had speedometer cables. The speedometer gets it's signal from the transmission computer.
Look up 1991 ford escorts manual transmission speedometer
A 1993 Jeep Grand Cherokee does not have a speedometer cable. The speedometer signal comes from a sensor on the transmission or transfer case.

To fix the inoperative speedometer you will need to replace the speed sensor. It is located on the side of transmission and has 2 wires coming out of it.
