It's an ionic bond because the electron from one atom are being given to the second atom so that it may have a full electron shell of eight electrons. Hydrogen is the only exception because it can only hold two electrons at the max.

How are the ions formed .?

Ions are formed in the environment by the transfer of electrons. This occurs in the atom when one electron transfers to another.

What occurs when an electron transfers from the outer shell of one atom to the outer shell of another atom?

I think you're talking about an ionic reaction. So a ionic bond is made as a result of that.

What is it called when chlorine becomes chloride?

When chlorine becomes chloride, the chlorine gains an electron to form an anion. This process is called oxidation. Oxidation always occurs with reduction (loss of an electron from another substance) in an oxidation reduction reaction.

What type of bonds occurs when an electron leaves an atom of an element and joins another atom?

Ionic

What is the transfer of that occurs when molecules bump into one another called?

Do you mean ionic bond? An atom with one extra electron can transfer its electron to an atom that needs an electron so that both atoms will have a full outer shell (valence). Then the two atoms are held together in an ionic bond.

What determines the extent to which a proton-transfer reaction occurs?

The weaker acid and base determine the extent to which a proton-transfer reaction occurs. Acid base reactions are called proton transfers.
