Killer Whales often hunt in packs. Killer whales probably tended to be bigger then they are today. Though they would not of needed to be in order to defeated the largest of Megalodon. Is some tactics such as latching onto the fins to manipulate path and tire Megalodon while the others ate away at the fins. all they really needed to do to kill it was tire and de-fin it.
I believe Killer Whales caused the final extinction of the last Megalodon's by hunting them down and killing them directly.
my rough theory:
First Megalodon's were reduced to the tropics by climate change. This reduction of the Ocean's top predator, allowed whales to experience prosperous times in the colder latitudes, more diversification and the emergence of the dolphin's and Orca's.
Eventually over time, and cultural evolution, led to the complex and varied social structures observed in them today.
Transient Pods probably evolved the strategies and tactics for hunting down and killing the Megalodon's in order to be able to hunt in new feeding grounds without themselves being the hunted. And would explain this strategy and aggressiveness towards large sharks that still exists today.
It's been observed that when a pod moves into an area today, if their are Great Whites in the area, they will kill them. (actually after 1 is killed the other great whites seem to all be aware of what happened and immediately head for safer waters)

A man that was five to six feet tall could step through its jaws without touching the mandila.
