Hole in the head is a common disease and is caused by a lack of certain elements/vitamins in their food. Most experienced aquarists feed their fish at least some live food every few days or so (at least once a week) to avoid this problem. Chopped finely or whole, depending on the size of the fish, and the size of the worm the best possible live food is earthworms. You would also need to be observing the basic rules of fishkeeping which are, :- 1 inch of fish needs a minimum of 1 gallon of water. :- Every tank needs a permanently running cycled filter. :- Every tank needs to have at least 50% of its water changed every week. Follow the above rules and your fish stand a chance of survival. Fail to follow them, and I can guarantee your fish will have constant health problems.
