An 'iron lung' is a machine that enables a patient to breathe, when the patient's respiratory muscles are paralyzed. It was invented by Dr Dinker and associates in 1928. A more efficient unit was made several years later. It is most famous for use during the polio epidemics in the 1940s and 50s, when literally thousands of people, many children, had polio. The device works by placing the patient inside, with only the head outside the machine. The machine then creates alternating negative and positive pressure to create 'breathing' for the patient. The negative pressure inside the unit inflates the lungs, and the positive pressure deflates them- ie inspiration and expiration. There are two ways to give oxygen to a patient. You can deliver it through a face mask, connected to a tank of oxygen, or you can place the patient in a room which has its own pure oxygen atmosphere, often at high pressure as well. High pressure (technically called hyperbaric) oxygen will enter the blood even if the patient is not breathing.
A urodynamic study with a point pressure leak test will allow a diagnosis to be made that can distinguish the patient who has a hypermobile urethra from the patient who also has ISD.
