Many things could cause this condition. Here's a small list of some common things:
Woman could not hold a farming job, cause they thought woman could not do that job.

It depends on the transmission, which you did not list.
Might want to pull off the inspection cover and see if the bolts that hold the torque converter are tight. Also check the condition of the flexplate.
It could be many things, loose or damaged shift linkage or build up of dirt in between the transmission housing or it's loose or out of alignment. More than likely what you are describing are bad syncros in the transmission. Syncros are brass rings inside the transmission that hold your gears in the proper selected range. when worn they allow your selected gear to pop out.
Only if its broke all the way around all the bolts that hold it on.
The amount of transmission fluid that a Ford F350 will hold depends on the size of the transmission. The average is about 19.2 quarts of transmission fluid. The specs can be checked in stores during purchase. Over filling will cause problems with shifting.
According to the owner's manual, the light is flashing because the transaxle has an electrical problem. Continuing to drive with it in this condition could cause damage to the unit.
How many quarts of automatic transmission fluid does the 2001 Toyota Avalon automatic transmission hold?
Our Kia has an automatic transmission, on the side of the shift lever there is a button. when going uphill you can push this button and it keeps the transmission from shifting out. When this is engaged hold appears on the instrument panel. Hope this helps.
6 bolts hold the transmission to the back of the engine.
"Hold" mode tells the transmission not to shift, or to "hold" a particular gear. This is useful occasionally when towing, or while climbing long, moderately steep grades. Continuing to drive in "hold" mode under normal conditions will cause extra stress on the engine and transmission, due to excessive RPM, or lugging the engine under too few RPM. You will also lose fuel economy, and power in some cases, depending on the gear you tell it to "hold".
