The Baitoushan Volcano last erupted circa 1702 and has been dormant since. Record keeping was spotty, but it is said that it spread pumice in a 25 mile diameter and erupted 25KM into the stratosphere in it's +/-969 AD, one of the largest eruptions in 2000 years.
The last time it erupted was in 1801. We don't have the statistics about the number of people that were killed, but it is said that it covered a whole village and a fishing pond.

There are three active volcanoes in Hawaii as of 2014. They haven't erupted since 1996. The volcano named Kilauea is said to be the most active in the word.
