It depends. Most storyline injuries are shown on TV e.g. Triple H being punted in the head by Randy Orton whereas real injuries generally aren't. Sometimes storyline injuries are to cover : a real injury, suspensions, time off or to create interest. So to answer your question some are real some are not. Surgery pics on wwe.com are real.
