The drain in the bathroom sink is 1 1/4 inch. However, the trap or the fitting on the wall may be 1 1/2 or 1 1/4 inch.

a home typically has a 4" main drain and that is enough. if it is not 4" you should get a plumbing code and do the math. Can you add a second 3-inch wasteline to an existing 3-inch main house wasteline in order to add the 3rd bath and toilet (this is obviously a 1950's house)?

A shower drain is 1 1/2 or 2 inch and a toilet needs to be either 3 or 4 inch.

All drains slope 1/4 inch per foot, minimum. Too much slope,though, can mean pulling water out of the trap or goose neck on a toilet.

Probably the wrong size flange. Is the flange a toilet collar? If so the pipe is probably 4 inch.
Depends on how much higher and how the toilet is made. They are not all the same. A PVC flange is supposed to be above the floor level and will clear. They are about 1/4 inch thick. You can probably get away with up to 3/8 of an inch, but any more than that and the toilet will rock on the drain and eventually leak.

Firstly, i strongly recommend against you reducing the pipe because you will pick up blockages, i have seen it far too many times. Secondly, check with your state laws regarding the minimum size piping for the toilet discharge oultet pipe, most countries it will be 4 inch or 110mm, hence, toilets made with a 4 inch outlet.
You can write this as 3/4, or as 0.75.
