That just means the disc between C4 and C5 is mashed out (like pushing down on a marshmellow) straight toward the middle of the spinal canal, where the spinal cord runs. Unless there are other abnormalities --such as bone-crowding of the canal as with short-pedicles-- this (0in many cases isn't something to worry about...
