Insurance is regulated at the state level. To provide an HMO in any state, not only do you have to develop a separate network to support the coverage, but meet the regulatory requirements to do so. States also regulate POS plans differently. So a company may base the plans offered in each state on regulations that must be met as well as what seems to fit the market demand.
