There are 12 States in the U.S. That requires ALL parties involved in the conversation (All Party Consent) to be made aware that the conversation is being recorded. These States are:

This falls under the United States Code: Title 18.2511. As long as you actually take part in the conversation and are doing so in a State that allows "One Party Consent", it is quite legal to record the conversation without another party in the conversation giving permission or even being told that they are being recorded.

Being taped w/o consent doesn't carry a penalty, that person is a victim - not a criminal. Setting up the taping, that's the crime - but I don't know what the punishment is.
Well, I don't think there is a single word used to convey this. Something to consider is that if you are in a public place, and provided you are not a minor, your consent is not required. Simply stated, if you do not want to be video taped, in military parlance, evacuate the AO (Area of Operation), beat feet, kick rocks, get out of the frame.

Why would you need someone else's consent to install a security camera on your home? After all it is for the safety and welfare of you and your child.
Are you talking about having your conversation taped by police? Basically, the rule is at least one person involved in the conversation has to know that the conversation is being recorded, otherwise there needs to be a search warrant. For example, if I were a police officer and I was talking to you, I could record the conversation without your consent or knowledge, because I am involved in the conversation, and I know it is being recorded. However, if I am a citizen talking on the phone to another citizen, a search warrent would be required because neither subject was aware of the recording being made.

Rarely is information available on when programs are taped or recorded and instead the information is released on when they will be aired. The Teacher's Tournament could very well have already been recorded and just not yet aired

No, it is generally not legal to videotape people having sex without getting consent from all the parties involved. The individual states have their own laws, and there is also a federal law the prohibits video taping someone (without their consent) under circumstances in which that individual has a reasonable expectation of privacy, regardless of whether the individual is in a private or public location. For Indiana take a look at Ind. Code Ann. § 35-45-4-5. Under that statute, "clandestine" or "surreptitious" video taping is considered "peeping". A prosecutor would only have to prove that a person on the tape was not aware they were being taped.
