You need an attorney to subpoena someone in court. This is not something you can do yourself.
Any court who has need of the records can subpoena them. It must be for a legal purpose. Which is to say, that your nosy neigbor or family members cannot subpoena them just for their own interest..
You should be able to get out of it if it is a true family emergency, however you would have to get permission from the court to do so. Another technique that is used to work around a witness's emergency is to take his testimony out of turn if it is at all possible, subject to being recalled if need be. There are ways these conflicts can be handled without simply hauling the witness to the courthouse regardless of his situation.
