With the engine cold, put it in the radiator just to be sure there is no air gap. Run it for a short time until it gets warm and coolant begins to circulate. Then install the radiator cap and put coolant in the overflow to the coolant level line.
A leak in a coolant system can usually be fixed by replacing a hose. If the leak is at the end of the hose, a new clamp might be necessary. If the radiator is leaking, the radiator might need to be replaced.
Because the motor is running to hot try flushing your radiator and putting new coolant in.
The radiator cap might need replacing.
If the smoke is coming from the electric fan itself, then it needs replacing. It could be the smoke is steam that the fan is drawing from the radiator, and you have a coolant leak at the radiator.

Low coolant? Radiator probably needs flushing and/or replacing after 40 years. Bad thermostat? System airbound? Water pump not circulating coolant? Hoses collapsing? Bad radiator cap?
Most, if not all, the coolant in the radiator reservoir will pour out when you pull the water pump. Drain the radiator and save the coolant (if you plan to re-use it) before removing the water pump.
