Of course it hurts! Do not crop dog's ears! Don't listen to them, you can have a dogs ears cropped. Obviously it does hurt them, but that is okay.

It shouldn't provided it is used properly. A dog whistle is a whistle that produces a pitch that is too high for most people to hear, but most dogs can hear. However, if you blow the dog whistle in the dog's ear, yes it will hurt the dog's hearing.
It's called a "metrenome", there are also stop watches now and metrenomes that clip to your belt and vibrate so you can feel it instead of hurt your ears with the annoying CLICK!. :) It's a cool idea for deaf drummers.

Once your dog is inside, check his ears again. Are they still cold? If so it may meet he has bad circulation or heart problems. If the dog has long ears he may have dipped them in his water bowl by accident. If the dog is sick, the cause of the cold ears could be that the blood and heat are all circulating around his major organs, rather than his ears.
