In severe cases, yes, but the swelling usually happens rather gradually such that the child will awaken if/when it becomes a real problem. If he's still asleep, it may create snoring or interrupted breaths, but is not likely to be a serious problem.

On the other hand, if you're worried about it, it's best to see a doctor.
