The half life of methadone is around 60 hours, so if you can stand to wait 3 days (72 hours) than that is better, the longer the better really. You also need to have been gradually coming down off the methadone and need to go down as far as possible. You must be below 40mg but if you can go all the way down to 10 or 15mg than that would make for a smoother transition. You need to be totally honest with your suboxone doctor about when the last time you used was so he can then tell you when to take the suboxone. Trust me you do not want to play around with this. I have seen so many people coming off methadone think " I'll just go ahead and take a suboxone" when it had only been like 24 hours, and the went into severe withdrawals immediately and where in pure HELL for a few hours. No matter how bad you feel just wait wait wait as long as you can. At least 48 hours.

Going from methadone to Suboxone will make you sick. Going the other way will not make you sick. However, long exposure to the narcotic antagonist in Suboxone will have made you far more sensitive to opiates like methadone. There is a good chance of overdose. The methadone should be phased in slowly, starting with much lower doses than normal. A better idea: get detoxed from the Suboxone if necessary, and get REALLY clean. suboxone has a long halflife about 23hours. day one you will feel no other opiates. day two you will feel them kick in then the suboxone stopping the opiate from working. day three you can begin to feel the full effect of other opiates. be careful not to take more trying to feel the other opiates. eventhough suboxone has naloxone in it, you can still overdose.
