You cannot buy codeine without a prescription in the U.S. but you can buy it in Canada and the UK over the counter. There may be other European countries but Im not sure which ones. You could probably get it in Mexico as well.

People used to be able to buy cough syrups containing codeine directly from the pharmacist... and based on their own diagnostic assessment. People born in the early 60's and earlier may remember having this ability. Presently, you may not diagnose yourself; every time you suspect you need codeine, you must get a new prescription from the doctor. So, no, you may not purchase codeine OTC anywhere in the United States.

IMPROVEMENT OF THE ANSWER ABOVE: Actually, you CAN buy codeine otc. However, what usually happens is it's put in with a whole bunch of other ingredients to deter anyone from abusing the drug. You can't get codeine pills period. However, you can get cough syrup with codeine in it. The only way to be able to do this is to go to a "Mom and Pop" Pharmacy. Independently owned pharmacy. You have to ask the pharmacist for it and the pharmacist assesses you and ultimately decides whether or not you NEED it. If he/she does see that you have a legitimate reason for needing it, you will be required to sign a log book with ALL of your information. Most places will sell you a 4 oz bottle every 48 hours. I wouldn't recommend going in every other day because the pharmacist will get suspicious and stop selling it to you.
