MOS is Military Occupational Speciality, and I believe the T is an indication that the service member is still in training, as opposed to MOSQ, which indicates that they are qualified in their MOS (i.e., have completed training in their career field).

If memory serves me correctly, the old 95B MOS was the MOS of an enlisted Military Police service member. That MOS is now 31B. The 95 series Career Management Field is no longer in use; ipso facto, the 95B MOS is no longer in use.
The Career Management Field is 68 (it was 91 when I first enlisted), and the MOS is 68W (again, it used to be 91B).

Pretty much anywhere where there are infantry, armour, or cavalry units to be found. But you did not give an MOS - you gave a CMF (Career Management Field). 13 is the CMF - MOS would be more specific (e.g., 13B).
Your MOS is selected when you sign your enlistment contract at the MEPS centre. While in BCT or OSUT, you may have the option to drop certain elective (for example, you can drop a RIP contract, or you can drop Airborne school), but you can't change your MOS. If you fail your AIT for the MOS for which you enlisted, then you'll be retrained in another MOS, although you're not very likely to have much of a say in regards to which MOS you're retrained as.
