These days, most hospitals do not keep original copies of such documents. They are obtained at City Hall or at the particular agency in your state tasked with providing certified copies of birth certificates. In some states, it is called the Registry of Vital Statistics, but it may have a different name in your state: a simple search online for "birth certificate" and the name of your state should turn up the correct agency. Many states have the form you need to fill out online. However, the birth certificate itself will not be free: there is usually a fee for a certified copy of it, and the amount of the fee varies.
