States set strict standards on who may or may not have sirens and emergency equipment on a vehicle. Any authorized emergency vehicle (ie. police, fire, etc.) would normally have a permit issued by the appointing authority in that state. As a volunteer firefighter in WV, we were allowed lights and sirens if we had a permit issued by the state. Failure to do so could result in charges for unauthorized equipment, or worse, impersonating an officer.

Yes, it is generally not legal for private citizens to have a siren installed in their vehicle. Sirens are usually reserved for emergency vehicles such as police cars, ambulances, and fire trucks to alert other drivers of their presence in urgent situations.
