For convenience. Although many ATM networks are widespread, no network can have a machine in every possible location. For example, I was recently in a small store that only accepted cash. Near the door, the store had an ATM that was not a member of any of the popular networks. Someone who was making a small purchase could choose to walk out of the store and go searching for an ATM on their network, or they could pay a fee to step over to the ATM in the store. I'm sure that some people will take the time to scout out an ATM on their own network, but I am sure that there are also people who may be in a rush or simply don't care to save the few dollars that the out-of-network ATM charges.

You would write to the bank or ATM company. Simply state you're interested in having one of their machines installed on your premises. They'll usually send someone over to survey the property - to see if (a) it's feasible to install an ATM and (b) that the premises are secure enough to deter thieves stealing the machine and/or contents.

It depends on your definition of the term "necessary." Ultimately it comes down to who is bearing the risk of theft - usually it will be shared between the insurer and the bank who owns the ATM. Most insurers will require CCTV systems on all ATM's (including ones in urban places). CCTV systems are so cheap and easy to install these days that it really wouldn't make sense for a bank to not install one near an ATM...
