Considering the number of books comprised of essays on the market I don't doubt you could publish a collection of them, although you will have to credit every single source you've ever used in that case. You will also have to find a publisher willing to put that book in print, and then possibly endure extensive lambasting from critics, depending on the field in which you are writing said essays.
If the source is available for public perusal on information databases you can get away with using the source without asking for permission as long as you cite the source from which it came, though this has probably already been beaten into your head repeatedly by whoever taught you to write academic papers or your essayist skills in general. You only need to get permission if the source states that request of such is required. It is probably polite to request permission anyway, regardless of whether or not it is required.
It is best practice to obtain permission from the original source for any information cited in your book, even if it is just a statistic. This helps to ensure that you are not infringing on the original author's copyright and protects you from any potential legal issues. If you are unsure about whether you need permission, it is advisable to consult with a copyright lawyer.

If you are using a Copyright Video and/or Audio, you need expressed written permission from the artist(s) before you can publish it. If you do not get this permission, and still publish it, you risk being sued for Copyright Infringement.
You have to get permission from the video game company before you can publish your videos on YouTube.
You will need your Scout Master's Permission and most likely you will need to get all the boy's parents permission if you have anything about them.

A permission to publish form is required when using materials reproduced from other works, particularly when held in libraries' special collections. Although associated with copyright permission, it can also address more esoteric requirements placed on the materials by library policy or even the works' donors.
To "publish" means to make information available for the public to view.
I could answer this but the information is private and not directly relevant. Matt is very private about family details, as none of them are involved in show business. It would not be fair to either Matt or his family to publish such information without permission.
Depends who you are writing about! If it's my Mum, then yes! You will need f*****g permission!!
