Nothing happens when you urinate while wearing a tampon.

Tampons are inserted into the vaginal canal, whereas urine comes out via the urethra, although the opening to the holes are both next to each other in the vestibule of the vulva they are still separate holes so urine doesn't get to the tampon. If using tampons with strings then the string can get wet, move the string out of the way and pat dry if it does get wet to avoid discomfort and risk of infection.

Yes it's absolutely okay to pee with a tampon in because you don't pee out of your vagina hole, there's a little hole where your pee comes out that's near your vagina hole.
