It was a rogue body captured by earth's gravity. This possibility is not given much credence today; earth isn't quite massive enough to have accomplished this.
It was sloughed off the earth as a huge bubble of material while the earth was molten and pliable. Considered now to be impossible. Extrapolating back in time, the moon's material would have had to be orbiting at an impossible velocity when separated from earth in order for it to be moving as it does today.
There was a collision between earth and a rogue planet, and this collision caused a splashing and rebounding of molten rock that eventually settled into what is now the moon. This is currently the explanation widely held to be the most likely.
