President Roosevelt (FDR) provoked the attack, knew about it in advance and covered up his failure to warn the Hawaiian commanders. FDR needed the attack to sucker Hitler to declare war, since the public and Congress were overwhelmingly against entering the war in Europe. It was his backdoor to war.
Yes. The US decoded the info hours ahead of time but the information got tangled up in bureaucracy... at least that my opinion on the subject. Even with the freedom of information act finding out such trivial things is hard.

No, the attack on Pearl Harbor was a complete surprise to the US government. But the USA had recently invested money in a new radar system and saw the Japanese planes on the screen but believed the machine was malfunctioning and giving false information. So the operator of the machine never sent out an alert.

The only people who knew for certain that Pearl Harbor would be attacked were in the Japanese military. On the other hand, war between Japan and the US was widely anticipated at least as far back as 1925, given American opposition to Japanese expansionism I suspect that most people anticipating a Japanese attack, however, would have expected it in the Philippines or somewhere else closer to Japan.
They intercepted a mesage from Japan to its naval fleet.
