Both. It depends on their genetic make up. In the North there would be more mixing of genetics with typically more fairer western European countries e.g. Germany. However there are also fair skinned people in Sicily. This is due to a mixing of genetics with other 'typically' fair skinned areas. Sicily has a history of being invaded by the Vikings and Normans- typically fair skinned people. This mixing leads to fair skin genes in the gene pool, hence the existence of fair skin Southern Italians.
In the north part of Italy, the ethnic groups are the Italians and small groups of Germans and French people. In the south, there are the Greek-Italians and the Albanian-Italians.
In real life the two sides of Italy hate each other because of their difference in wealth. The Southern Italians hated the Northern Italians because they had loads of money while many places in the south live in poverty like conditions; and the North hates the South because it makes them look bad. In reference to Hetalia, the author seems to have only focused on Southern Italy's jealousy of North Italy's wealth.
