Usually you shouldn't need to express your dogs anal glands manually as the dog will generally do this on their own. Anal glands are expressed naturally when your dog has a bowel movement. If your dog has been scooting on the floor, biting their anus or if there is a rash or swelling of the anus then take your dog to the vet. A infection will not be erradicated by expressing glands manually...this only makes it more comfortable for the dog and does not treat the infection. Medication will be required.

When they have bowl movements the muscles in the anal area express the glands, and sometimes little dogs have problems with that so the need to have them done manually.
