This sounds like an ignition problem. Check distributor cap and rotor, coil wire, spark plug wires, spark plugs, timing and firing order.
You have a warped rotor. It must be trued at a machine shop or replaced.
Your front brake rotors need to be machined or replaced.

Running hot and shaking can be 2 separate problems. Running hot is caused by, in addition to what you have already done, defective water pump, blown head gasket, or a radiator that is clogged really badly, either internally or externally. The shaking can be a vacuum leak. The vehicle is 14 years old. Replace every vacuum hose under the hood. Replace one at a time as to not get confused. Also check the brake booster for a busted diaphragm.
Rotors out of round - need to be re-surfaced or replaced
You need the front rotors machined or replaced.
Could be in need of a tune up.
