they concluded the season with Zuko asking his father where she is, and I'm guessing it might play a part in a potential spin off... if people keep asking the producers, then maybe they'll just put it in the legends of korra. They shouldn't have left it so "mysterious."

It is never made clear why Ursa left, but it can be inferred that she was forced to betray Ozai, the Fire Nation, or both in order to save Zuko from being killed by Ozai.

You never find out. It shows Zuko asking his father but we're never told the answer, which is really sad but also nice because it's never good to know everything.
It can be assumed that he will eventually, but it is never said, nor are the whereabouts of his mother.
On his father's side, Zuko had one cousin, Iroh's son Lu Ten. We don't know if he had any cousins on his mother's side.
Unfortunately, Zuko likes Mai, though we all know someday him and Katara are going to get married and live happily ever after!
