Only in a Church; exception is if other party is Jewish or other religion that, out of respect for their faith, the church allows marriage any place at all, provided paperwork is proper and other party commits to all the requirements of a valid catholic marriage (permanent commitment, children to be raised catholic, free to marry,etc.)

Normally, it does; one may get a dispensation however to hold the ceremony elsewhere, for example in a garden or in the courtyard of a castle. This may especially apply if one of the spouses is a non-Catholic.

You can't. Canon Law specifically prohibits it. The Catholic Church only recognizes marital union "inside the body". If a Catholic priest marries a couple outside the Church he is disobeying his vows. If however, he has a license to marry, the state has no opinion whatsoever and cannot. If the two are married outside the Church their marriage is thus voided by the Catholic Church and is blasphemous. However, a Catholic priest may, as a friend of the couple, attend their wedding. they cannot represent the Catholic Church. Its that simple.
