If you are a Puerto Rican citizen, you are also a United States citizen; this means you are born with basically the same rights as someone born in the United States. You don't need a passport or Visa, just a valid license or identification or sometimes a birth certificate.
If you're a US citizen you do not need a visa to visit Puerto Rico. However, if you're not I suggest verifying the requirements in the tourism information website.

Yes you do. Puerto Rico is a "Estado Libre Asociado" from the United States of America. A visa will be required in the same fashion you would need one to go to Miami.

Puerto rico is a terretory of the United States, you do not need a passport to go there.

Puerto Rico use same electric systems as US continental.
Yes
No, you do not need a passport to enter or exit Puerto Rico if you are a citizen of the United States. Puerto Rico is a territory of the USA.
As long as you are traveling directly from Puerto Rico to NY, without stopping in any foreign countries - for example, elsewhere in the Caribbean or in Canada - you do not need a passport to travel. Puerto Rico is a US Commonwealth. Travel to or from Puerto Rico and/or the US Virgin Islands to the US mainland is travel within the United States and passports are not required. You will, however, need a government issued photo ID, such as a drivers license.
You do not need any special documents such as a passport to go to Puerto Rico because it is considered a US territory. All you need is some sort of identification.
